Ilion, N.Y. — Three employees at a Herkimer County nursing home have been indicted after a 34-year-old resident who required constant supervision choked on a meatball and died, according to the state Attorney General’s Office. The resident was staying at The Grand Rehabilitation and Nursing at Mohawk in Ilion and had been transferred there from a rehabilitation hospital three days before her death on Aug. 23, 2025, the AG’s Office said in a news release Thursday. The resident had a rare genetic condition that caused tumors throughout her body and made it difficult for her to swallow solid food. She also had an eating disorder that caused compulsive eating, the AG’s office said.
